Police Record Check

All employees & volunteers at The Oxford School are required to provide a Police Record Check. Police Record Checks must be completed once every five years, each subsequent year an Offence Declaration form must be completed. If an Offence Declaration lapses a new Police Record Check will be required.

To complete your Police Record Check, obtain a signed “Request for Police Record Check” form from TOS administration, during regular business hours (Monday to Friday 9:00AM - 3:30PM). After obtaining a sign Request for Police Record Check, the check can be submitted using one of the below options. Police checks can take anywhere from two to six weeks to be processed.

 

Residents Outside of Woodstock

Applications for police record checks can be submitted online.

HAVE the following ready for your application:

  • A “Request for Police Record Check” signed by the Head of School, requesting the Vulnerable Sector check, from TOS administration

  • Be prepared to answer financial questions in order to verify your identification (as part of the EIV process - Electronic Identity Verification).

  • No payment required for volunteers


Woodstock Residents

Applications for police record checks can be submitted online or in person.

Record checks typically take 5 business days to process.

Please bring the following with you:

  • A “Request for Police Record Check” signed by the Head of School, requesting the Vulnerable Sector check, from TOS administration

  • 2 pieces of identification, including 1 government-issued photo identification (see below for a list of identification options)

  • A form of payment for the $15 fee (credit, cash or debit)

  • Proof of residency within the city of Woodstock

ACCEPTABLE FORMS OF PHOTO IDENTIFICATION:

  • Passport

  • Driver’s License

  • Canadian Citizenship Card

  • Permanent Resident Card

  • First Nations Status Card

  • Military Family ID Card (MFID)

  • Nexus Card

  • Ontario Identification Card, issued by the MTO

  • Ontario Photo Health Card

ACCEPTABLE FORMS OF NON-PHOTO IDENTIFICATION:

  • Birth Certificates

  • Immigrations Papers

  • Certificate of Live Birth

Social Insurance Number (SIN) Cards are not permitted as an acceptable form of identification.
